C2C provides a full range of primary care services across the lifespan. From preventive, perinatal, and pediatric care, to treatment for acute illness, long-term care for chronic conditions, and diagnostic services, our team delivers care through clinics, in homes, and across our communities. Our patients return to C2C for all of their healthcare needs because we deliver consistent, high-quality service that they can trust.
Community Health
Our committed team of community health workers (CHWs) extend C2C’s services directly into the communities and homes of our patients. All C2C clinics have dedicated teams of CHWs who perform outreach, providing health education and information about C2C’s services, conducting home visits to reduce loss-to-follow-up, screening for non-communicable diseases, and referring patients to the clinic as needed. CHWs drive demand for C2C’s clinical services, support rural and hard-to-reach areas, and most importantly, build trust between C2C and our communities.



Maternal Health
Our Maternal Health Program provides continuous support through pregnancy, delivery, and the postnatal period for both mother and newborn. At C2C clinics, nurses and midwives provide prenatal check-ups, counseling, and ultrasound exams so patients know how their pregnancy is progressing and have the information they need to make informed decisions. We have also launched our Mothers Club initiative across our network, which offers social support to women at similar stages of pregnancy and education on best practices for healthy pregnancy and caring for newborns. Our maternal health staff encourage women to seek facility-based birth to improve their chances of safe labor and delivery. Following birth, our maternal health staff visit mothers at home to ensure they are healing well and that their babies are safe and healthy. We want every woman in our communities to feel supported, informed, and cared for.



Sexual and Reproductive Health
Our Sexual and Reproductive Health Program provides adolescents and young adults in our communities with clear, honest health information and resources. We work with schools and communities to implement a comprehensive sexual and reproductive health education curriculum with the overall goal of reducing the rate of unintended pregnancies in our communities. Most importantly, we seek to empower our participants with the knowledge and resources they need to make decisions that affect their bodies, and ultimately, their futures. We want girls and boys to feel comfortable in their bodies and know that it is up to them to decide when to start a family if they choose to and that they are worthy of an education, of a healthy relationship, and a happy life. C2C’s clinical staff also offer participants private counseling, family planning, and STI testing in a safe, respectful space.



Malnutrition Care
Our Malnutrition Program helps families care for children who are experiencing malnutrition by screening for malnutrition, providing treatment, such as ready-to-use therapeutic food, and educating parents and caregivers on how to prevent relapse, including holding cooking demonstrations to teach about proper nutrition and how to prepare healthy recipes. CHWs follow up with families at home to make sure children improve and fully recover. Through building strong connections with our community members, we work together with families to ensure children can get and stay healthy.
